service fees
The Commission's Office enforces the following fees for customer services offered (effective as of November 9, 2020). The fee schedule is required to be reviewed bi-annually by the Commission. Both the Commission and its office maintains full discretion to change or revise its fee schedule at any time when deemed necessary.
Restrictions Apply:
Restrictions are fully applied to services requesting any copy of the claimant's record, file, or documentations. All workmen's compensation files under the Commissioner's custody are treated as confidential. Both the Commissioner and the Office Head of the Commission's Office maintains full discretion to deny any request for the claimant's file or record unless such records are summoned or requested by the Administrative Law Judge, or any of the courts allowed by law to adjudicate workmen's compensation claims.
Request to obtain any of the claimant's records or file are only confined to the bona fide claimant (employee) or petitioner, the respondents (employer and carrier), and their respective represented legal attorneys. Requests by any governmental entity, authority, or official to access or obtain such file or records are also restricted unless such entity or person is a bona fide respondent within a claim.
No copy of a claimant's record shall be considered authenticated unless certified by the Commissioner, or the Office Head of the Commission's Office, as a true copy of the original.
