PHYSICIAN'S FIRST INJURY REPORTING

First Reporting of Injury following First Treatment:

It is mandated by law for the employee's Primary Treating Physician (PTP) or doctor to furnish and provide a medical report upon first treatment to both the Commission and employer. No claim for medical or surgical treatment is considered enforceable against an employer unless the employee's treating or primary physician submits within 20 days a first report of the injury after the first treatment or examination.

To facilitate such reporting, the physician must use and submit Form ME-01: Physician's First Medical Report of Injury after First Treatment. It is important for the treating physician or doctor to attach and enclosed his full medical report with the form upon submission. It is also the responsibility of both the employee and his treating physician to ensure a first reporting is submitted to the Commission's Office.

The Commissioner may excuse the failure to furnish such reporting form within 20-days if he finds it to be in the interest to do so, and he may upon application by a party in interest make an award for the reasonable value of such medical or surgical treatment so obtained by the employee.
